Forensic Futures Strategic Plan Progress Report

HealthShare Ltd, on behalf of the Midland DHBs has commissioned a project to conduct a service review of progress made against the Midland Forensic Futures Strategic Plan (2008-2013), so that seamless delivery of service occurs between providers and across service and geographic boundaries.

Perinatal Infant Continuum Phase 1 Report

More women and their families in the Midland region are affected by perinatal and infant mental health disorders and addiction. This is not just a Midland region issue but consistent with other populations nationally and in other parts of the developed world. Burt and Quezada (2009) estimated that 15 per cent of pregnancies are affected by some form of maternal mental disorder.

This project has been established to increase knowledge and understanding of the Midland region’s perinatal and infant mental health, and addiction services needs.  A stock take was undertaken which confirmed that mental health and addiction services across the five DHBs in the Midland region had referral processes in place to support women with perinatal mental health disorders. This was specific to accessing secondary level assessment and intervention. Infant mental health services were less developed in the smaller DHBs and more often integrated with general paediatric service provision. Services for children who had parents with a mental health or addiction issue were being provided in one DHB only.

Consumer Competency Framework

Currently there is no nationally consistent competency framework for service user workers or any professional body accountability or code of conduct. This causes a lack of consistency in how service user workers are expected to work. The lack of national baseline competency frameworks and guidelines for employment and best practice hinder the development and quality consistency of this workforce.

To this end, a project was commissioned by He Tipuana Nga Kakano the Midland Consumer Leadership Network to start developing a framework utilising a bottom up approach. This work will feed into the Tri-party project (Te Pou, Northern Regional Alliance and Midland) that is being undertaken.

The purpose of this project was to:

  1. Develop a service user and peer support competency framework

  2. Provide a Midland perspective to the Tri-party project with Te Pou, Northern Alliance and Midland Share Services

  3. Gain endorsement from the Midland DHBs to the developed framework

  4. Feed into the MoH Service Development plan strategies

Family Whānau Competency Framework

Following a strategic planning sessions facilitated by Fred McRae in October and November 2012 each of the regional networks identified a key strategic objective for the 2013 / 2014 Mental Health and Addiction Workplan for the Regional Service Plan.

The Midland Whānau Leadership Network (Te Ao Whānau) identified the need to establish a competency framework for family whānau workers in mental health and addiction. Nationally, little work has been undertaken in this area despite the fact that family and whānau workers have been part of the mental health and addiction workforce since 2008.

The project led by Sue Lewer commenced in May 2013 with consultation forums being held in each of the five Midland DHB areas. A further two Maori specific consultation hue led by Hine Moeke-Murray occurred in Tairawhiti and Taranaki to develop Maori for Maori competencies. A draft competency document was developed from the consultation feedback and was placed on the programme for final consultation at the October 2013 Whānau Summit: “An Unskilled Workforce….Yeah Right!”

Workforce Strategic Plan 2011-2014

In December 2012 the Mental Health Commission (MHC) released the Blueprint II and the Ministry of Health (MoH) released the Mental Health and Addiction Service Development Plan (SDP), “Rising to the Challenge”. The impetus of both documents for mental health and addiction services is to increase national consistency focusing on access, service quality and outcomes for people who use services for their whānau and for the communities that they live in. Both documents set a service development pathway with clear actions and expectations for the next five years.

Summit Reports from 2013

The Midland Family Whānau Summit was held on the 15 & 16 October followed by the Midland Consumer Summit held on the 19 & 20 November.  We shared each of the summits with Family Whānau workers, family members, consumers, support workers and interested participants from across the Midland region who came along to be part of the two day events.

Mini Conference Evaluation Report

In August and September 2012 the Midland MH&A regional forums held their one day Mini-Conferences in Rotorua.  These conferences were open to the Mental Health & Addiction Sector to attend the following:

  • Consumer Leadership network – He Tipuana nga Kakano network: “Building Consumer Leadership”

  • Alcohol and Other Drugs network: “Creative Futures for Addiction: Making it Happen”

  • Whanau Leadership – Te Ao Whanau: “Families are Fundamental”

  • Nga Purei Whakataa Ruamano “Whanau Ora: Recognising CAMHS and Primary Mental Health”

Co-existing Enhanced Practitioner Two Day workshop

In June 2012 the Midland MH&A Network initiated contact with Matua Raki and Te Pou proposing further development and implementation of a training package specifically focusing on Enhanced Practitioners to ensure local development and sustainability.  The two day workshop was facilitated by Dr Fraser Todd, Ashley Koning, Suzette Poole with support being provided by the Midland Workforce Coordinator.

Services for Older People's Continuum of Care Project

This project was commenced in 2011 to support Midland region DHBs in their planning and service development to address the needs of a growing older population. Consistent with the Ministry of Health’s Guideline on Mental Health and Addiction of Older people and Dementia (2011), an integrated and coordinated approach was sought to improve access to services, and the quality of services provided.

DHBs provided data on current service provision, consistent with the Seven Tiered Model (Draper, Brodaty & Low, 2006), in response to a devised questionnaire. This data were used to demonstrate the gaps in service provision and areas where needs were being addressed. Further to this, a swap shop day was held with key stakeholders to discuss and debate what was working well and where further development was warranted. This information together with a scan of the literature has been collated in this report.

This information provided a foundation for a set of draft recommendations to be developed for implementation. Key stakeholders were invited to critique them at a further swap shop day.

The recommendations included:

Pathway and Service Model Development

  • Develop regional pathways and service models for the service continuum that span primary, secondary and tertiary care, consistent with the Draper, Brodaty & Low’s Seven Tiered Model, that are integrated and person and whānau centred, commencing with a regional dementia care pathway.
  • Pathway development will aim to provide clarity about what services including diagnostics, and models of care are required to meet the person and their carers’ needs from pre-diagnosis to advanced care.
  • Attention will be given to the linkages, interfaces and risk management of primary, secondary and tertiary providers working collaboratively to deliver care at the right level in the right place by the right health professional.
  • The client group with multiple co-morbidities resulting in a complexity that doesn’t fit well with local service provision will be identified and regional options investigated. For example, mental health clients who experience dementia/significant cognitive decline in relation to their mental illness, people under the age of 65 who experience cognitive decline due to neurological disorders, and dementia, and people with intellectual disability who also experience mental illness or dementia.


  • Support the identification of learning competencies required by staff and people caring for older people accessing services in the Midland region, and recommend training that includes: dementia, delirium, depression, managing complex clients with multiple co-morbidities, elder abuse and neglect, legislation, PPPR Act, and cultural perspectives in ageing.

Takarangi Competency Framework 2011

Since 2009, the Midland Workforce Coordinator has been responsible for implementing the Takarangi Competency Framework workshops across the region.  This competency framework focuses on enhancing staff's ability to work with and alongside Maori.  The Takarangi Competency Framework is linked to the Midland Region Mental Health and Addiction Strategic Plan 2009-2015, the Midland Workforce Development Plan 2010-2014 and the Midland Co-existing Problems Competency Framework 2011.

The 2011 implementation was undertaken as a project with the Midland Workforce Coordinator leading the project in partnership with Matua Raki (National Addiction Workforce Centre).  A special thank you to Ruth Choudharey for her tireless work to ensure that the 2011 roll out was well coordinated and beneficial to the participants.

Alcohol & Other Drugs Clinical Qualification Guidelines

This project was commenced in March 2011 led by Pam Armstrong.  Following sector wide consultation the Alcohol and Other Drugs Clinical Qualifications Guidelines was developed.  It was signed off by the General Managers Planning and Funding in February 2012.  A frequently asked and answered questions sheet was developed to assist providers.  The team would like thank the Midland Region Addiction Forum for providing support and advice to the project consultant to ensure that the finished document was comprehensive and complete.

Clinical Governance Project 

A project was undertaken across the Midland region led by Deirdre Mulligan.  The project was to determine a clinical governance model that will support the way forward in mental health and addiction in the Midland region.  Following extensive consultation the project report was signed off by the Acting Chief Executive of HealthShare, Cathy Cooney with agreement to progress the recommendations.  The team would like to thank the project Steering Group for their support to ensure that the comprehensive document was completed.

Phase II of the project is in the process of being undertaken.

Midland Eating Disorders Strategic Plan 2008-2015

The MR Eating Disorders Plan was developed using a Eating Disorders Reference Group comprised of members from across the Midland region and across the primary, secondary and teritary sector.  The Midland plan was signed off by the Ministry of Health in August 2009.

Needs Assessment Project 2011

The Midland Region Needs Assessment 2005 was completed prior to key data being available to the region. For example, Te Rau Hinengaro and PRIMHD data. Further to this, the Midland Clinical Services Plan has been developed and excludes mental health and addiction. These factors have contributed to the need to re-do the Midland Needs Assessment to ensure that it is current. 

Joan Mirkin was contracted to undertake the project and was supported by a Steering Group to ensure that the report was relevant to the Midland mental health and addiction sector. 

Midland Region Mental Health & Addiction Needs Assessment 2005

The Midland regional plans presents a vision, aims and objectives that will guide the DHBs in the Midland region, and all other key stakeholders, over the next ten years. The plans are supported by a detailed and comprehensive Midland Needs Assessment Service Coordination Project Report May 2011.

Co-existing Problems Strategic Plan 2010-2015

A regional approach was undertaken to develop the Midland Co-existing Problems (CEP) Strategic Plan 2010-2015 which was signed off by the Ministry of Health in January 2011.  Taranaki, Lakes, Bay of Plenty and Tairawhiti DHBs contributed to its development.  Waikato DHB elected to develop a separate plan.

DHB specific Implementation Action Plans for the four contributing DHBs are included in the plan.

Lets get real - Midland NGO Evaluation Forums

The Midland Regional Network secured funding to conduct five NGO Lets get real Supporting People, Supporting Change Workshop Evaluation forums that were undertaken in November - December 2010. These workshops will be hosted by a regional provider and facilitated by Ruth Choudharey with support from Eseta Nonu-Reid.

The purpose of the workshops is to identify any issues/barriers of using the LGR Enablers in each region and how this information can be used to assist the development of further resources to support the implementation of LGR in the NGO organisations.

Midland Mental Health & Addiction Strategic Plan

The MR MH&A Strategic Plan 2005 has been re-written to include clear alignment to Te Tahuhu's 10 Leading Challenges.  The plan has been widely consulted and was signed off by the Midland GMs Planning and Funding and Maori Health in November 2009.

Midland Region Maori Health Plan 2009-2015

The MR Maori Health Plan 2009-2015 provides high level strategic direction to improve responsiveness to Maori.  This is a key document for the Midland mental health and addiction sector.

Forensic Futures Strategic Plan 2011-2015

The MR Forensic Futures Strategic Plan was developed in consultation with the Midland adult mental health and addiction sector and was signed off by the Ministry of Health in August 2009.  The plan is relevant to Taranaki, Waikato, Lakes and Bay of Plenty DHBs.  Tairawhiti DHB receive Forensic services from Capital and Coast DHB.  As identified in the MR FF plan six monthly Stakeholder reports have been developed and will be published on this page.
